• Service Plan effective upon receipt of payment; Member benefits effective upon activation of plan.
• Service Plans 𝐚𝐮𝐭𝐨𝐦𝐚𝐭𝐢𝐜𝐚𝐥𝐥𝐲 𝐫𝐞𝐧𝐞𝐰 𝐞𝐚𝐜𝐡 𝐲𝐞𝐚𝐫; the Card on File will be charged upon acceptance and each calendar year thereafter, starting one calendar year from the date of acceptance. Service Plans renew upon receipt of cash or check payment.
• Service Plan and Member Benefits applicable for one (1) system located at the address referenced on this agreement.
• Visits to be scheduled and completed during normal business hours; scheduling subject to availability; Availability reserved for emergency service June 1st – October 31st or where weather permits. Visit schedule subject to adjustment, where applicable.
• Service Plan non-transferrable; Service Plan may be canceled in writing by either party within 30 days of acceptance. Refund applicable for visit(s) not completed prior to cancellation of Service Plan.
• Replacement parts subject to availability; Service Plan coverage subject to becoming void as a result of obsolete parts and/or part availability.
• Service Plan applicable for units identified by serial number; Service Plan null and void upon replacement of equipment.
• Service Plan subject to Terms and Conditions, where applicable.

*Manufacturer Warranty valid with documentation of proper maintenance on equipment.
